Online Booking and Estimate Request Forms for Repair Shops
Many customers would rather request an appointment at 10pm than call during the day. A short, clear form can bring in jobs you'd otherwise miss, as long as it's easy to finish on a phone and someone follows up quickly.
Keep it short
Ask only what you need to call them back or book the job. A good starting set:
- Name
- Phone (and email if you send confirmations)
- Vehicle year, make and model
- What's going on (a free-text box)
- Preferred day or time (optional)
You can ask for mileage, VIN or detailed symptoms later. Every extra field means fewer people finish. web.dev's form guidance makes the same point: remove fields you don't need.
Make it phone-friendly
- Use the right field types so phones show the right keyboard:
type="tel"for phone,type="email"for email. - Turn on autofill (
autocompleteattributes likename, tel, email) so people can fill in their details in one tap. - Big fields and a big submit button with clear text: "Request appointment".
- Give every field a visible label. See keyboard and form accessibility.
Set expectations
- Say what happens next: "We'll call you within one business day to confirm."
- Show a clear confirmation message after submitting, and send a confirmation email or text if you can.
- For estimates, be honest that a final price may need an inspection.
Behind the scenes
- Test it monthly: submit a test request and make sure it actually arrives. Broken forms that silently fail are common.
- Use HTTPS so personal details are protected (Do I need HTTPS?).
- Ask only for what you need and say how you'll use it.
- If you use shop management software with online booking, check whether it can embed directly on your site.
Check your own site in seconds
Our free website check for auto repair shops looks at one page for broken links, Google basics, phone setup, HTTPS, image descriptions and business details for Google, and explains what it finds in plain English.
